I needed a gift card for my niece's birthday to take with me today - so I whipped this one up really quick. These are really quite simple to make.

Insert into your crimper - with the open side facing you - turn your crimper about 4 clicks. This will help secure the bottom of your gift card holder.
Using your 1 1/4" circle punch, punch a 1/2 circle into the top of your gift card holder. This will allow the recipient to easily remove the gift card. Finish gift card as desired.
